11.9 Annual Review of Student Progress.

The graduate program in which a student is seeking a graduate degree will provide a mechanism for assessing the student’s progress toward degree completion at least once annually. If it is determined the student is not to be making adequate progress, then a specific plan to address and correct any inadequacies in progress will be prepared in a written document provided to the student and the dean of the Graduate College annually by June 30 [By completing this form]. Failure to correct these inadequacies may result in termination from the graduate program and/or Graduate College.

Timeline:

Annual reviews should be completed by June 30th each year.
In the case of unsatisfactory progress, the student must receive written documentation of the issues and expected remediation. The documentation should be submitted (uploaded) below.
The graduate program provides the Graduate College with an aggregate summary of all reviews (i.e. complete the form below).
